Inglewood & Districts Health Service (IDHS) is an integrated rural health service nestled in the peaceful rural hamlet of Inglewood in country Victoria.  Offering a broad range of services including acute care, community health and hospital and residential aged care to more than 5,500 residents in the Shire of Loddon, and located less than an hour from the regional centre of Bendigo, IDHS is offering a very attractive option to work in a rural setting while having access to all the facilities and services of a large regional centre.

Diabetes Educator – Part-time (0.8), Fixed term to 30.6.2018

The position of Diabetes Educator is funded under the Early Intervention in Chronic Disease Program (EiCD) and is designed to provide a centre-based, rural outreach Diabetes Education service.  The main responsibility of the Diabetes Educator is to provide education, advice and support to children, young people and adults – and their families – living with diabetes.

The Diabetes Educator is responsible for expertise in diabetes management and will ensure people with diabetes and their families are well supported within the hospital and in their local community.

All appointments to IDHS are subject to a satisfactory police check.

 Key selection criteria                                          

  • Bachelor of Nursing or equivalent
  • Registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse in the State of Victoria and holding a current practicing certificate
  • Post graduate qualifications in Diabetes Education
  • Ability to work as part of a multi-disciplinary team as well as independently
  • Demonstrated high level of inter-personal skills with the ability to effectively liaise with patients, clients, the community, service providers and other agencies
  • Understanding and/or experience of the community health needs of people in a regional setting
  • Understanding and/or experience with evidence-based chronic disease management
  • Demonstrated effective written and verbal communication skills including report writing skill

Mental Health Worker – Part-time (0.4), Fixed term to 30.06.2018

The Mental Health Worker will provide support and advocacy to people with a mental health issue.  The position will support clients’ lifestyle choices and assist them to access community resources, to better manage their mental health symptoms and to identify strategies to enhance their everyday living skills.  The position will provide information to clients and advocate when necessary to ensure equitable access to community resources.  The position will work collaboratively with the other services to ensure best possible client outcomes, including developing collaborative care plans and care coordination.

 Key selection criteria                                          

  • Certificate IV in Mental Health / Alcohol and Other Drugs / Community Services or demonstrated experience in Mental Health Services or similar
  • Knowledge of mental health issues, illnesses and disorders as well as developed knowledge of regional health and welfare service providers
  • Proven ability to empathise with people who experience mental health issues
  • Ability to develop individual program plans
  • Proven ability to work unsupervised and effectively manage own time
  • Proficient with computers including the use of Microsoft Office
  • Demonstrated experience in undertaking intake and assessment activities and understanding referral pathways
  • A satisfactory police check
  • A Working with Children Check
  • A current Victorian Driver’s License
