  • DescCardiac rehabilitation is an exercise and education program that supports you while recovering from a heart procedure or problem. Cardiac rehab has been proven to improve quality of life and prevent readmission to hospital. It is a crucial step on your road to recovery.   • Pulmonary rehabilitation is an exercise and education program that supports you while recovering from a lung procedure or problem. Pulmonary rehab has been proven to improve quality of life and prevent readmission to hospital. It is a critical step on your road to recovery. The program runs all year round and consists of 8 weeks at our Inglewood campus.

  • Diabetes Educators are diabetes specialists. They are health professionals who have completed further studies to focus on helping people with diabetes self-manage their diabetes effectively and prevent complications. They can see anyone with diabetes and their families, including newly diagnosed or previously diagnosed, women who develop diabetes during pregnancy and people at risk of developing diabetes.

    A diabetes educator can be the first point of call when you want more information, support and motivation in managing your diabetes and the link between other health professionals. They have in-depth knowledge of all aspects of diabetes. They can recognise when you need to see other healthcare team members, for example, an optometrist or podiatrist.

  • DescriptioThis group is an 8-week program that our Exercise Physiologist and Diabetes Educator conducts. A referral is required from your GP, and an assessment by both our Diabetes Educator and Exercise Physiologist is completed before the group. Exercise is a great way to control and manage diabetes (and many other health conditions), with the advantages well researched.n text goes here

Self-Referral

Please click here to obtain a self-referral form. This form can be printed and completed before sending to our referral email, fax or delivered to Inglewood Hospital front reception.

Copies are available at Inglewood Hospital front reception if you do not have a printer.

Fees

Many programs are free.

Some programs attract a small co-payment assessed against your income. This is collected by self-income declaration on admission to our services.

Failure to Attend and Cancellation Policy

We understand that things happen and life gets busy, but to help IDHS manage our waiting lists and to keep your care consistent, we require more than 24 hours' notice if you cannot attend an appointment. Your circumstances will always be considered, but when people do not attend their appointments, it can delay your care and the care of those on the waiting list.

We offer free SMS reminders about all appointments. If you are not receiving them and you would like to, please speak with reception or your therapist.

Initial Needs Identification

Once we receive your referral, one of our intake workers may contact you to ask additional questions regarding your health. This is to help identify if there are any other services (either here at IDHS or another provider) that you may benefit from. This is a free service and is completed via telephone.
